When you ring to make your first appointment with Dr. S.M. Pereira, we will request your details including your preferred email address and the name of your referring GP. We will also request to know your method of payment (i.e. if you have private medical insurance or are self funding your treatment). If you are with a private medical insurer (PMI) we will ask you to seek prior authorisation from your insurer, for funding of your initial assessment and of any subsequent treatment, as your PMI will insist on this as per their procedure. You may also choose to fund your own treatment without going through your PMI. The first appointment is the initial assessment and therefore requires you to fill out a registration form along with a few screening questionnaires, for you to bring along to your assessment.
The duration of initial assessment is about 45minutes to approx 1 hour.
